Datatel/SPIRIT Request Forms

You can download SPIRIT related forms here. Please send the completed forms to the ITS HelpDesk in Hennenberry Hall.

• Request for Datatel Access - This form is for new employees who will need to log into Datatel as part of their job duties.  Must be signed by the employee's supervisor, and in some cases also by the Executive Director of Enrollment Services.

• Request for Datatel Security Changes  - This form is for employees who already have Datatel access but need to make changes to the list of Datatel screens they are allowed to access.

• Request for Datatel/SPIRIT Misc Services - This form is for members of the community who wish to request changes, ad hoc reports, or other services related to the Datatel/SPIRIT system.

• Request for Contractor/Volunteer Access - This form is for members of the community who wish to request various levels of SPC network access for the contractors and volunteers they are working with

• Report Packet - This form is for users who are requesting custom Datatel/SPIRIT reports and need to define the output format very precisely.  (Most of the time the "Request for Datatel/SPIRIT Misc Services" form will suffice and you will only need this "Report Packet" form if suggested by an ITS technician.)

• Report Layout Guide - This is an additional detail form that can be used (if needed) in conjunction with the "Report Packet" form
