New, Freshmen, Transfers:

• Physical Form packet completed and signed

• Sickle Cell Form completed and signed

• Copy of Primary Insurance Card (front & back) or Copy of school bill w/ Health Insurance Fee paid

 ReturninG:

• Physical Form packet completed and signed
  

  Physical Form for Returning Student-Athletes

• Copy of Primary Insurance Card (front & back) or Copy of school bill w/ Health Insurance Fee paid

 **All Men’s Soccer, Women’s Soccer, Men’s Basketball, and Women’s Basketball new, freshmen, and transfer participants must complete concussion baseline testing to be arranged with the Sports Medicine staff for medical clearance.

 **All documentation, testing, and proof of insurance must be submitted by deadline for medical clearance to be granted. No exceptions.
